De Baca County Record Book Instructions
(Books may be handwritten or typed, by members) Copies of forms and/or Disks available at the Extension Office
*COVER Use a standard record book cover obtained from the County Extension Office; 3-ring notebook binders also acceptable.
*PICTURE PAGE Mount one 2x3 or 4x5 photo of yourself on the first page. Use rubber cement or glue to mount. Below the photo, put the following information: NAME, ADDRESS, TOWN, STATE, ZIP CODE & CLUB NAME.
*DIVIDERS(optional, but a good organizational idea) Use dividers with tabs that will recess inside the record cover to prevent tabs from tearing in handling during. Tabs go in front of the material referred to by the tabs.
*TABLE OF CONTENTS On a single sheet of paper, list the principal sections of your record book. Use the following order: 4-H Report Form, 4-H Story, Project Record Sheets and Project Photos (one page of project photos follow each set of project record sheets); Non-project 4-H Photos; Miscellaneous 4-H related news-clippings and correspondence. Page numbers not necessary.
*DE BACA COUNTY 4-H REPORT FORM must be completed by all members and signed.
*4-H STORY-- 4-H story is an essay of your 4-H experiences for this year. Your story is limited to three pages written on one-side only (double-spaced for those who type). For novice members, the story should be a minimum of one half page.
Presentation of Story: In your story, introduce yourself and emphasize the human interest side of your 4-H projects and activities. Summarize your leadership, citizenship and community service experiences as well as how your projects have helped you develop future plans and goals. Tell what you liked most about 4-H this year, what was most challenging, etc. Check it for grammar, spelling and punctuation. Write in complete, coherent sentences and paragraphs.
*PROJECT RECORDS AND PROJECT PHOTOS --Each project taken should have a record sheet that includes: Goals, Accomplishments (what you learned related to projects), Opening/Closing inventory, Monthly Activities and an Income/Expense report. Members are encouraged to use the De Baca County Project Record Sheet for all projects (including livestock) unless specific projects taken have their own unique record forms such as rodeo, sewing, photography, This Is 4-H, etc. You may have one page of project photo per project, immediately following each record sheet. This is optional and should highlight growth experiences including captions. (If you have more than one project, use dividers between the projects, with the dividers labeled as what the project is. Make sure the project record sheet and project photos are together behind one divider).
*GENERAL 4-H PHOTOThese photos should highlight the 4-H Activities you took part in, OTHER SPECIFIC PROJECT WORK. 1 to 3 pages may be used. Examples, county/district/state contests, Teen Getaway, Clean-ups, etc.
*4-H RELATED NEWS-CLIPPINGS--This is your place for 4-H related news-clippings and correspondence. Do not include pins, ribbons, or metals. This section is optional and will not be judged. Please put in a plastic sleeve. This assists with book organization.
